RENTAL FEES

Table Loom: $30.00 per month (plus $1.88 tax) for each month in which I have not taken two (2) or more lessons. The fee shall be payable on the first of the month for that month. No charge otherwise.

Floor Loom: $50.00 per month (plus $3.13 tax) for each month in which I have not taken two (2) or more lessons. The fee shall be payable on the first of the month for that month. No charge otherwise.

I agree to use reasonable care to protect the condition of the loom and I agree to be responsible for loss/damage in the amount of up to the current, wholesale replacement cost of the loom and/or repair.

To rent a loom please print and return this Agreement Form with your payment.
